***Membership Terms and Conditions:

1. Membership Period: This membership contract is valid from day of purchase till canceled. It is subject to renewal as per the terms specified herein.
2. Membership Benefits: As a member of Cultivate Wellness Med Spa, you are entitled to the benefits and services outlined in your chosen membership tier. Any additional services or discounts offered as part of your membership will be specified separately.
3. Membership Fees: You agree to pay the membership fee of your chosen membership Tier on a Monthly basis. Payment is due on the day of purchase of each Month.
4. Payment Methods: Payment can be made by credit card, bank transfer, cash. Automatic payments may be set up for your convenience.
5. Cancellation Policy: Should you wish to cancel your membership, you must provide written notice at least 30 days prior to your next billing cycle. Membership fees are non-refundable.
6. Membership Termination: Cultivate Wellness Med Spa reserves the right to terminate your membership at any time for violation of the terms and conditions outlined in this agreement or for any other reason deemed necessary by the management.
7. Privacy Policy: Cultivate Wellness Med Spa will handle your personal information in accordance with our privacy policy, which can be found on our website.
8. Changes to Terms: Cultivate Wellness Med Spa reserves the right to modify the terms and conditions of this membership agreement. Any changes will be communicated to you in writing, and you will have the option to accept the changes or cancel your membership without penalty.
9. Governing Law: This membership agreement is governed by and construed in accordance with the laws of California. Any disputes arising from this agreement will be subject to the exclusive jurisdiction of the courts in California.
