I authorize SA IV Center to charge & keep my credit card on file for the monthly membership payment amount on the initial payment date and for the same amount each month thereafter. I acknowledge that no prior notification is needed unless the date or amount of charge is subject to change, in which case SA IV Center must notify me of such changes at least 30 days in advance of the change(s).
